Essential Functions

  1. Conducting psychosocial assessments and program intakes along with the Director Social Services for clients of the Social Services programs.
  2. Gather and track client data for programmatic outcomes and ensure that client charts are thoroughly documented.
  3. Ensure that client paperwork is updated regularly and in compliance.
  4. Regularly communicate with program clients to ensure program satisfaction, address challenges, collect data, inform of policies or updates, and provide support and referrals as needed.
  5. Assist clients to access appropriate benefits and basic needs; appropriate medical, mental health and substance use treatment providers.
  6. Provide crisis intervention for immediate housing needs, medical, mental health and substance use issues.
  7. Assist Director of Social Services with direct care staff management including interview and hire process, providing orientation and guidance, reviews, and coaching and disciplinary action when needed.
  8. Assist the Social Services department with scheduling and coordinating staff for in-home visits as well as multiple center-based locations as needed.
  9. Assist with any community inquiries on all agency services, client referrals to community resources, and represent the program at occasional community events and/or presentations.
  10. Coverage of emergency cell phone on a bi-weekly basis or as needed.
  11. Plan and execute monthly Social & Activity Group for program.
  12. Assisting the Social Services Administrator with social service-related programs and events such as nutrition education classes, parent groups, sibling groups and social work team meetings.
  13. Assist with supervision, task management and instruction of social work interns.
  14. Prepare parent surveys.
  15. Organize and maintain client files and staff as required by the County grant.
  16. Assisting with data collection, monitoring and reporting requirements related to enrollment and social services.
  17. Flexibility of working hours will be required to meet the child and family’s needs which include evening (2-3 per week) and weekend hours as needed.

Certificates, Licenses, Registrations

Valid Florida Driver's License.

First Aid/CPR Certification.

Education and/or Experience

Minimum Bachelor’s in Social Work, Non-Profit Management or related field, and/or 1-2 years social work experience if BA is in unrelated field.
