Minimum Qualifications:

Bachelor's degree in Psychology, Social Work or Mental Health / Behavioral Science. Criminal Justice degree considered with 15 hours of coursework in psychology, social work and/or counseling with 2 years of MH case management experience

Job Description:
This position is responsible for providing case management, social and habilitative services, and counseling to patients. Works under the supervision of a Psychologist, Mental Health Manager or Program Director.

Job Duties:

• Triage calls received by UTMB’s CPAN team.
  
• Fulfill resource requests.
  
• Refer calls to providers for consultation.
  
• Enroll clinics and providers in CPAN and PeriPAN.
  
• Contribute to the resource database for all projects.
  
• Liaisons with patient, patient families, on-site departments and community agencies to coordinate current treatment and/or assist with aftercare planning.
  
• Create and develop mental health education materials as assigned.
  
• Attend project related meetings and participate in project related activities.
  
• Adheres to internal controls and reporting structure.
  

Marginal or Periodic Functions:

• Performs related duties as required.

Salary Range:

• Commensurate with experience.
