*This is a non-benefited, per diem/casual position with day shifts. The schedule will be around 10-20 hours per week, between the operating hours of 7am and 7pm, Monday through Friday. This is a work-from-home position, but requires residency in San Diego. Some training may be on-site.*

Scripps Health is nationally recognized as a leader in endocrinology and diabetes care. The Diabetes Care Manager - RN will oversee care management for diabetic pregnant patients.
Responsibilities include:
* Works independently to manage a panel of high risk patients in close collaboration with endocrinologist/perinatologist.
* Under physician guidance, provides expert clinical education & management specific to the patient's risk criteria, including insulin titration instructions as well as insulin pump adjustments prn.
* Enters medication & DM supply refill orders as needed.
* Assesses patient's needs and places referrals to other care team members, such as endocrinology, perinatology, behavioral health as needed.
* Participates in care coordination conferences w/provider partners.
* May develop population health strategies for specific high risk groups, such as T1D, pregnancy, under-served populations (FQHC).
* May provide transition support across both ambulatory and inpatient settings to insure successful transitions of care.


*Required Education/Experience/Specialized Skills:*
* Minimum of 3 years of experience working in one of the following areas/roles: RN in endocrinology, perinatology, obstetrics, women’s health clinic, diabetes clinic, community clinic.
*Required Certification/Registration:*
* RN and CDCES certification
*Preferred Education/Experience/Specialized Skills/Certification:*
* Bilingual English/Spanish, Insulin Pump Trainer certification, specific experience care managing a population of patients with diabetes.
* The ideal candidate will have a strong clinical background, including 3 or more years of experience working with patients with diabetes to include complex patient populations such as: DM in pregnancy, Type 1 Diabetes, etc. In addition, knowledge and experience managing patients with diabetes technology such as insulin pumps and continuous glucose monitors is imperative.
