Coastal Hospice invites you to join a team of caregivers who are committed to promoting dignity and quality of life for patients and families who face life-limiting conditions. In your role as Palliative Care Team Social Worker you will:

  • Meet with potential patients and families to discuss palliative care services and how the program could meet their needs.
  • Conduct Advanced Care Planning with patients and their families.
  • Provide emotional and psychological support to patients and families when needed.
  • Connect patients and families with community resources and support services.
  • Act as a liaison with local referral sources.
  • Provide a consistent high-quality experience for every patient and family served by helping to identify patient and family goals.
  • Provide patient and family counseling when identified necessary by palliative care providers.
  • Communicate needs and other information in a timely and effective manner with the Palliative Care Team, providers, patients, families and community resources.
  • Introduce the concept of hospice care with patients and families when appropriate.
  • Participate in weekly departmental team meetings.
  • Provide education on the Coastal Palliative Care program to the community and referral sources when requested.
  • Maintain continuity and consistency of care by:
    • Recording all pertinent information in a concise, effective manner according to organizational standards
    • Completing documentation in the patient’s electronic medical record by the end of the assigned work day
    • Providing hand off communication according to established policy and procedure


  • Master’s degree in Social Work from an accredited school
  • 2 years’ social work experience preferably with varied community resource knowledge and contacts
  • Current LCSW or LCSW-C Social Worker license in the state of Maryland
