Job Summary:
We are seeking a dedicated and compassionate Clinical Social Worker to join our interdisciplinary healthcare team in a Long-Term Acute Care Hospital with both medical and psychiatric units. The ideal candidate will apply clinical social work techniques in collaboration with physicians, psychiatrists, psychologists, and other healthcare professionals to address and alleviate social, emotional, and environmental barriers that impact patient recovery and care outcomes.
Key Responsibilities:
- Apply clinical social work practices in the diagnosis, treatment planning, and therapeutic support of patients in a hospital setting.
- Address patients’ psychosocial, emotional, or environmental barriers through individual counseling or group therapy.
- Participate in diagnostic and treatment conferences as part of a multidisciplinary team to ensure a holistic care plan.
- Provide crisis intervention, discharge planning, and aftercare coordination for patients transitioning out of hospital care.
- Educate and counsel patients and families to help them cope with illness, mental health conditions, and care decisions.
- Conduct psycho-social evaluations, develop care plans, and maintain accurate documentation in accordance with hospital policy.
- Collaborate with external community agencies and connect patients with resources including housing, mental health services, and rehabilitation programs.
- Offer consultation and support to clinical staff (including nurses, therapists, and physicians) regarding patient social needs and care coordination.
Requirements:
- Master’s Degree in Social Work (MSW) or a related field such as Counseling.
- Active state licensure as a Licensed Clinical Social Worker (LCSW), Licensed Independent Clinical Social Worker (LICSW), or Licensed Mental Health Counselor (LMHC) (based on state requirements).
- Previous experience working in a clinical or hospital-based setting, preferably with both medical and psychiatric patients.
- Strong knowledge of community resources and psychosocial issues affecting patient care.
- Excellent communication, critical thinking, and crisis intervention skills.
- Ability to work independently and as part of a multidisciplinary treatment team.
Preferred Qualifications:
- Experience in long-term acute care or forensic psychiatry is highly desirable.
- Bilingual or multilingual abilities are a plus.
