This position is responsible for building a strong, compassionate caregiver team and ensuring seniors receive the highest level of personalized, non-medical care.

Key responsibilities include:

  • Sourcing, interviewing, and onboarding mature caregivers (typically 50+)

  • Conducting client inquiries, assessments, and care planning

  • Matching clients with compatible caregivers and maintaining service quality

  • Acting as the central point of contact for both caregivers and clients

Position Summary

We are seeking a dedicated and resourceful Care Coordinator to serve a hybrid role that includes caregiver recruitment, client intake and screening, and care coordination. This role is vital to building a strong caregiver team and ensuring that seniors receive thoughtful, personalized care. It’s ideal for someone who thrives on building relationships and takes pride in making meaningful connections between clients and caregivers.

Key Responsibilities

v Client Screening & Intake

  • Respond to new client inquiries with empathy and professionalism

  • Conduct phone screenings and in-home assessments to evaluate care needs

  • Educate clients and families about services offered and the onboarding process

  • Document care requirements, preferences, and relevant personal details

  • Collaborate with management to determine appropriate care plans and service levels

v Caregiver Recruitment & Onboarding

  • Develop and post job ads on platforms like Indeed, Facebook, and local boards

  • Actively source and screen candidates—primarily mature adults 50+

  • Conduct interviews, reference checks, and background screenings

  • Organize caregiver onboarding, training, and documentation

  • Maintain an active pipeline of quality caregivers ready for client matching

v Care Coordination & Support

  • Match clients with caregivers based on compatibility and care needs

  • Schedule initial introductions and support the onboarding process

  • Maintain ongoing communication with clients and caregivers to ensure satisfaction

  • Resolve concerns or conflicts with sensitivity and professionalism

  • Document updates, changes, and follow-ups in scheduling or CRM tools

Qualifications

  • Experience in home care, healthcare coordination, customer service, or HR/recruiting

  • Strong interpersonal and communication skills, especially with older adults

  • Organized, self-motivated, and able to manage multiple priorities

  • Tech-savvy (Google Workspace, scheduling software, spreadsheets, etc.)

  • Empathetic, dependable, and community-minded

  • Valid driver’s license and reliable transportation for in-home visits

  • Must pass a urinalysis drug test

Preferred Qualifications

  • Background in caregiving, social work, or client services

  • Familiarity with elder care issues and local senior resources

  • Experience working with or recruiting adults 50+

Work Environment & Schedule

  • Hybrid: Combination of remote work, community outreach, and occasional in-home client visits

  • Flexible part-time hours; occasional evening or weekend availability may be required
