Overview
We are seeking a dedicated and compassionate Care Manager to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care, ensuring that individuals receive the appropriate services and support they need throughout their healthcare journey. This position requires a strong understanding of case management and medical terminology, as well as experience in various clinical settings such as ICU, Emergency Room, and Trauma Centers.
Responsibilities
- Develop and implement individualized care plans for patients based on their specific needs and medical history.
- Coordinate with healthcare providers, including nurses and specialists, to ensure comprehensive patient care.
- Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.
- Conduct assessments to evaluate patients’ physical, emotional, and social needs.
- Facilitate communication between patients, families, and healthcare teams to promote understanding of treatment options.
- Manage utilization of resources effectively to enhance patient care while minimizing unnecessary costs.
- Maintain accurate documentation of patient interactions and care plans in compliance with healthcare regulations.
Skills
- Strong knowledge of case management principles and practices.
- Familiarity with medical terminology relevant to various clinical settings including ICU, Level I and II Trauma Centers, clinics, and emergency medicine.
- Experience in utilization management processes to ensure appropriate patient care.
- Excellent communication skills for effective collaboration with healthcare teams and patients.
- Ability to assess complex situations quickly and make informed decisions regarding patient care.
- Compassionate approach with a focus on improving patient outcomes through coordinated care.
