Our Elderly Services Program (ESP) helps older adults remain safe and independent in their homes. This is a 36 hour/week, full time and benefits eligible role. We believe strongly in a work-life balance for our employees. The Care Manager Assistant (CMA) will provide support to the Care Management (CM) Director, CM Management Team, and Care Managers. The CMA position may involve Home Visit Intervention assistance, and telephonic and in-person support. This position will require usage and navigation of data entry software and will be responsible for entering case notes and understanding billing processes. The CMA must demonstrate the ability to work independently as well as maintain a thorough knowledge of Elderly Services Program policies and procedures, community and funding resources, and will obtain specialized knowledge of the services requested.
Primary Duties/Responsibilities:
- Complete documentation of all communication and tasks between clients, providers and community members.
- Complete CM Interventions as requested by CM Director.
- Provide support for Care Management Team and Intake Department as needed.
- Provide office support as requested by CM Director.
- Maintain understanding of all funding sources, including Medicare, Medicaid, waiver programs, etc.
- Educate and/or assist clients regarding public benefits.
- Attend staff meetings and mandatory trainings.
Potential Additional Duties:
- Provide back-up support for other roles such as Intake, billing, care management phone support, consumer-directed care support, environmental services, home modification services and/or home medical equipment services.
- Conduct home visits to obtain signatures for needed forms, environmental safety photos, benefit applications, commodity box deliveries or related activities as requested by CM Director.
- Complete mailings for Care Management staff and Management team.
- Participate in Outreach activities as needed.
- Other duties as assigned.

Education and Experience:
- Required: Associate's degree in human services field and at least two years of experience in home health care, medical social work, geriatrics, care/case management or an equivalent substitute.
Physical Requirements:
- Prolonged periods of sitting at desk and working on computer.
- Requires ability to work in homes that may have environmental concerns such as hoarding, pest problems and/or unsanitary conditions.
- Requires ability to ambulate in various housing venues including using steps and/or climbing stairs in different housing sites.
- Must be able to lift up to 15 pounds.
