Rocky Mountain Care is looking to hire someone that has attention to detail, is well-organized, self-motivated, independently driven and reliable for a full-time care coordinator position in our Woods Cross office as part of our Palliative team.
Preferred Qualities:
Previous CNA or Medical Assistant experience preferred but not required.
Demonstrates respectful interpersonal and communication skills with patients and colleagues alike.
Ability to create and maintain Microsoft Office documents in relation to the job.
Competent with phone communication to schedule and confirm appointments.
Respond professionally to requests and inquiries about the services the practice provides using practice guidelines.
Ability to multi-task and prioritize duties while managing interruptions.

Summary
We are seeking a Care Coordinator to join our team at Rocky Mountain Care. In this role, you will play a vital part in ensuring that our patients receive comprehensive care tailored to their unique needs. Your contributions will help us maintain our commitment to high-quality patient outcomes.
Responsibilities
- Coordinate patient care activities and ensure seamless communication among healthcare providers.
- Assist in the development and implementation of individualized care plans for patients.
- Maintain accurate patient records and documentation in compliance with healthcare regulations.
- Facilitate appointments and follow-up visits for patients as needed.
- Act as a liaison between patients, families, and medical staff to address concerns and provide support.
- Monitor patient progress and adjust care plans as necessary to achieve optimal outcomes.
Requirements
- Proven experience in a healthcare setting, preferably in a coordination or administrative role.
- Strong understanding of medical terminology and healthcare practices.
- Excellent organizational skills with the ability to manage multiple tasks effectively.
- Strong communication skills for interacting with patients, families, and team members.
- Experience with medical administrative support is highly desirable.
