Position Summary

The Pediatric Care Coordinator supports the delivery of high-quality, patient-centered care for pediatric patients at CrescentCare. This role focuses on assisting families with navigating healthcare systems, addressing barriers to care, and linking patients with internal and external resources. The coordinator works collaboratively with clinical and case management teams, community partners, and referral agencies to promote access, efficiency, and continuity of care. Fluency in medical Spanish and the ability to work across multiple sites are essential to this role.

Key Duties And Responsibilities

      • Develop and maintain relationships with referral partners and community-based organizations to support care coordination.
      • Interface with families of pediatric patients to assess needs and support care navigation including pre and post visit outreach.
      • Connect patients and families with internal CrescentCare resources such as case management, referral coordination, legal, and eligibility teams.
      • Coordinate with hospitals and other healthcare providers in conjunction with the case management team.
      • Assist in scheduling pediatric visits, managing referrals from outside agencies, and addressing barriers to appointment adherence.
      • Participate in the design and implementation of workflows that promote efficient and high-quality pediatric visits, including well-child care.
      • Accept and manage pediatric referrals from government agencies, schools, and community partners.
Qualifications

Education

Associate’s or Bachelor’s degree in a healthcare or social services-related field required.

Other (licensure, Certifications)

  • RN licensure preferred but not required at start.
  • Fluency in medical Spanish required.
