JOB SUMMARY
The Mental Health Professional (MHP) is responsible for conducting initial assessments for patients seeking behavioral health services. The role includes gathering comprehensive information on mental health history, substance use, and psychosocial factors to support appropriate treatment planning and referrals. The MHP plays a critical role in providing a welcoming and supportive environment for clients accessing behavioral health services.
JOB DUTIES
Intake & Assessment:
- Conduct initial intake interviews to gather detailed information on mental health history, substance use, and psychosocial factors.
- Work on a desktop system to schedule patient appointments.
- Document all information accurately in Centricity or other electronic medical records systems.
- Provide immediate crisis intervention as needed.
- Collaborate with Behavioral Health Consultants (BHC), Care Coordinators, and community resources to ensure seamless client care and follow-up.
- Refer clients to internal or external behavioral health services when necessary.
- Offer empathetic and supportive communication to clients and families during intake.
- Educate clients about available behavioral health services and treatment options.
- Schedule initial appointments and coordinate necessary follow-ups.
- Participate in regular team meetings, case consultations, and training sessions to enhance service delivery.
- Work with clinical and administrative staff to improve intake processes and client outcomes.
- Provide mental health education and outreach to the community as needed.
- Registered Medical Assistant or Certified Community Health Worker (CHW)
- Training with 2-5 years of clinical experience (Behavioral Health experience preferred).
- Ability to administer CPR.
- Valid Illinois driver's license, insurance, and personal vehicle for client transportation as necessary.
- Willingness to work weekends and overtime in emergency situations.
Full-time position based on clinic operating hours.
May require some evening or weekend hours depending on client needs.
PHYSICAL REQUIREMENTS & WORKING CONDITIONS
Office-based and community outreach settings, with occasional travel required.
Moderate physical activity, including lifting up to 25-50 lbs.
Extended periods of standing, walking, or sitting as required by job duties.

COMPENSATION & BENEFITS
Salary: $45,000-47,000

Occupational Risk for Exposure to TB:
There is an occupational risk for exposure to TB, as this position involves extensive community and patient contact. Employees in this role are required to comply with annual TB testing as part of workplace health and safety protocols. The only acceptable exceptions to this requirement are documented medical contraindications or religious objections. Employees who decline TB testing will be required to wear a mask when deemed necessary.
