Job Description:
Terrace Home Care is seeking a bilingual Home Care Coordinator to join our team in Brooklyn, NY. The ideal candidate will have at least one year of experience in home care intake, with the ability to manage the intake process efficiently.

In this role, you will play a key part in ensuring our clients receive top-quality care by managing schedules, coordinating services, and supporting caregivers.

Responsibilities:

• Schedule and coordinate home care services for clients.
• Communicate with clients, families, and caregivers to address needs and concerns.
• Maintain accurate records of care plans and service schedules.
• Ensure compliance with agency policies and state regulations.
• Assist with recruiting and onboarding caregivers as needed.

Qualifications:

• Experience in home care, healthcare, or administrative roles preferred.
