Job Summary:
Location: Methodist Hospital
Address: 8303 Dodge St, Omaha, NE

Work Schedule: 24 hour per week between the hours of 8:00am-4:30pm, Mon-Fri

Provides administrative support to Care Management Team.

Ensures on the job training to be eligible for a Social Work position.
Responsibilities:
Essential Functions
Assessment
  • Assessments to include identification of pre-hospital level of functioning and current patient/family utilization of community resources (Meals on Wheels (MOW), Home Health Care (HHC), etc.)
  • Utilizes standard form for assessments.
  • Refers patients to MASH when appropriate.
  • Communicates patient needs to the post-acute care provider.
  • Identifies need for interpreter and communicates to department secretary.
  • Aware of community services and placement options available.
Discharge Planning
  • Communicates with patient/family options for post-acute care both private and community based and arranges same.
  • Collaborates with health care team on required medications, durable medical equipment and other discharge needs, makes appropriate referrals.
  • Arranges appropriate transportation for patient at dismissal.
  • Notifies patient, family, and care team of final discharge arrangements.
  • Referral as appropriate to Adult Protective Services (APS), Child Protective Services (CPS).
Patient/Family Teaching
  • Communicates with patient/family options for post-acute care both private and community based.
  • Communicates with patient/family benefits/limitations of provider for post-acute care.
  • Interdisciplinary Rounds as needed
  • Shares pertinent information with Utilization Review and other members of the health care team if not in attendance.

Job Requirements
Education
  • Bachelor’s Degree or actively enrolled in a Masters of Social Work program required.
  • Active or enrolled in a Social Work program and will graduate within 24 months required.
  • Participates in mandatory in services and continuing education programs as mandated by policies and procedures/external agencies and as directed by supervisor.

Experience
  • Six months of social work or direct health care experience in a health care setting required.
  • Experience working with the elderly population and families preferred.

License/Certifications
  • N/A
