Compensation
Mount Sinai South Nassau provides a salary range in good faith determination of potential compensation to comply with the New York State law on Salary Transparency in Job Advertisements. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.

Department: Transitional Care Unit
Position Summary
Summary:
Under the direction of the Director of Social Work, the social worker will provide psychosocial assessment/evaluation of patients admitted to Transitional Care Unit. The Social Worker will provide direct social work services to patients and families, as well as, participate in formulating and implementing discharge plans, identifying unmet psychosocial needs and developing programs to address those needs.
Qualifications
Responsibilities:
Education Required
•Must complete in-house orientation program
•Master's in Social Work (MSW) from a school of Social Work accredited by the Council on Social Work Education (CSWE).
Experience And Skills
•Excellent interpersonal skills
•Speaks, reads and writes English
•Prior experience in long-term care setting required.
•Knowledge of case management preferred.
License/Certifications
•Licensed Master Social Worker or Licensed Clinical Social Worker.
