- Title of Position: Bilingual Care Manager
- Position Type: Full-time
- Salary Range: $50,000-$65,000 per year (Depending on Degree. Additional compensation for licenses and language skills)

- Department/Program: Champions for Children’s Health
Essential Functions:
- Client-Centered Care: Collaborate closely with children, their families, and the care team to provide tailored care management and support.
- Needs Assessment: Conduct initial and ongoing assessments (CANS-NY) to determine the most effective care plan, and input data into the NYS online portal.
- Plan of Care Management: Develop and oversee individualized care plans, ensuring timely and effective service coordination based on the child’s needs.
- Service Coordination: Facilitate access to services, support adherence to treatment plans, and monitor overall care including prevention and wellness.
- Home Visits: Conduct regular face-to-face visits with clients, ensuring consistent and comprehensive support.
- Documentation & Compliance: Maintain accurate records using the Medicaid Analytics Performance Portal (MAPP) and Electronic Health Records (EHRs).
- Advocacy & Education: Act as an advocate for clients, educate families on managing chronic conditions, and promote preventive care.
- Resource Coordination: Identify and connect families with community resources, manage referrals, and support transitions from hospital to home.
- Professional Development: Engage in ongoing training to enhance your skills and stay updated on best practices.
Required Skills:
- Bilingual
- Experience in child welfare, developmental disabilities, mental health, or healthcare systems.
- Proven ability to coordinate services, link clients to resources, and provide advocacy.
- Excellent communication, organizational, and collaborative skills.
- Passion for supporting children and families with special needs.
- Ability to travel within assigned areas and maintain compliance with agency policies.
