• Job Title: Travel – Master Social Worker (MSW)
• Location: Fort Pierce, FL 34954
• Shift: Day Shift – 8:00 AM to 4:30 PM (40 hours/week)
• Contract Length: 13 Weeks
• Pay Package: $2,000 – $2,150 per week (based on experience)

Join a 344-bed acute care hospital committed to patient-centered care. This role supports a dynamic care coordination team and directly impacts patient outcomes by facilitating safe and timely discharges and continuity of care planning.

Unit Details

• Number of Beds: 344 (excluding Behavioral Health and Acute Rehab)
• For discharge planning only, caseload ranges from 25–30 patients
• Documentation Systems: MIDAS, Meditech, Nate, Navihealth, Care Team Assignment
• Call Requirements: Must be available to work every third weekend
• Floating Policy: All social workers float except those in trauma
• Patient Experience Focus: Strong skills in hospital discharge planning and interdisciplinary team (IDT) collaboration

Required Qualifications

• Active MSW (Master of Social Work) or LCSW (Licensed Clinical Social Worker)
• Acute care hospital case management and discharge planning experience
• Proficiency in MIDAS, Meditech, Navihealth, PASRR
• Ability to manage complex discharge coordination and collaborate with care teams

Preferred:

• Familiarity with Medicare/Medicaid discharge regulations and transitional care planning
• Effective communication and problem-solving with families and multidisciplinary staff
• Adaptability to float across units as needed (excluding trauma)

Typical Responsibilities & Duties

• Facilitate and document discharge planning for patients across diverse units
• Conduct patient and family consultations and coordinate post-acute referrals
• Participate in interdisciplinary rounds (MDR/IDT)
• Ensure compliance with payer requirements and regulatory standards
