- Provide service coordination to individuals in the HCS Program, including support to families, through regular face to face and telephone contact to coordinate services that meet the needs of individuals.
- Facilitate person directed plans to assist the individual with achieving goals and determining the supports needed to meet those goals.
- Link individuals and families to community resources.
- Monitors the delivery of and individual’s/LAR’s satisfaction with all HCS services and non-HCS services and reports progress/lack of progress made from the perspective of the customer/LAR on all outcomes included on the PDP every 90 days 100% of the time.
- Meets 100% productivity targets for contacts each month.
- Provide accurate and timely documentation of all service coordination activities and responsibilities including contacts, Person-Directed Plans, Plans of Care, Financial Assessments, and other documentation according to current HHSC, H,CS and HOTBHN policies and procedures
- Participate in staff development activities in relation to assigned duties and according to current program requirements, including peer chart reviews.
- Performs related work as required.
Experience working with individuals with an intellectual disability is preferred.Education
Position requires a high school diploma with at least 2 years of experience with individuals with IDD, an Associate’s Degree in a Human Service Field, or a Bachelor’s degree in any field from an accredited college or university.KNOWLEDGE, SKILLS AND ABILITIES:
- Must be able to effectively interact with consumers, families and service providers to maintain an adequate plan for each consumer on the caseload.
- Must be able to consistently and in a timely manner produce documentation as required by the HCS rule and billing guidelines.
- Must be flexible in schedule yet organized to keep up with demands of individual consumers and program requirements.
- Must be able to monitor the effectiveness of the services received by the consumers and communicate concerns to providers.
- Physical capacity may include prolonged sitting, necessitate carrying supplies up to 15 pounds, driving vehicles to transport consumers and/or their families and attend meetings at various sites.
- Local travel 30%, Regional travel 40%.
- Applicant must be able to successfully participate, perform, and complete the skills required for CPR, First Aid, and PMAB certification.
