Telehealth Counselor

As a Telehealth Counselor, you will oversee all aspects of a patient’s treatment. Responsible for the coordination and continuity of care from admission through discharge and follow-up of the patients. You will also promote performance improvement and service delivery by providing comprehensive psychological therapy directed towards addressing the causes and effects of the patient’s addiction.

- CDCA or higher certification or licensure in Ohio
- Licensed Social Worker or higher licensure
- Licensed Professional Counselor or higher licensure
- Licensed Marriage and Family Therapist or higher
- Experience with dependency and/or medication assisted treatment.
- Ensure the completion of patient databases, treatment plans, progress notes, discharge summaries, and other essential documentation in accordance with applicable federal and state standards and agency policies and procedures.
- Provide patients with orientation regarding objectives of the program, phase system requirements, rules and regulations, sanctions, hours of services, patient rights, and additional information to help patient adjust to the program.
- Ensure each patient is assessed and placed according to the treatment phases/ levels of care in accordance with Federal and State standards and agency policies.
- Keep all patient files organized and updated for own and agency’s use in accordance with established policies and procedures.
- Provide counseling to patients in crisis situations by identifying patient resources and coping abilities to help them cope with and take steps to resolve the crisis.
- Engage with patients who refuse to comply with specific program requirements with the goal of increasing patient awareness of consequences of nonadherence.
- Facilitate referrals to other agencies and resources in the community when appropriate.
- Perform treatment plan reviews.
- Participate in case conferences and case reviews in accordance with established policies and procedures.
- Develop and increase knowledge of federal, state, and agency regulations regarding confidentiality, medication assisted treatment, and substance use counseling certification through annual required trainings, continuing education, and other educational opportunities as assigned.
- Submit external correspondences such as letters and progress reports to community partners [e.g., parole/ probation officers, case workers, etc.] on behalf of the patient to the supervisor for review and approval prior to sending to ensure communications are accurate and consistent and not solely based on patient requests.
- Secure necessary documentation from patients, when appropriate, to ensure compliance with established policies and procedures.
- Submit monthly and quarterly statistical reports to supervisor as assigned.
- Attend supervision meetings as scheduled.
- Attend team meetings and complete all training courses as required.
- Other duties as assigned.
