Valley Professionals Community Health Center is looking to hire a RN Care Manager for our Crawfordsville location.

Base salary of $61,800-$83,470

Job duties:
This position will work directly with patients providing health education and working with the patients to improve health outcomes. The RN Care Manager will manage high risk patient care, including the management of patients with multiple co-morbidities, and will coordinate the continuity of patient care with external healthcare organizations.

Requirements:
Must have current Indiana licensure as a Registered Nurse
Prior health education experience preferred
