A well-known hospital is seeking a compassionate and dedicated licensed Social Worker (LMSW or LCSW) to join its Inpatient Medicine Department. In this role, the Social Worker (LMSW / LCSW) will be responsible for identifying and addressing the psychosocial needs of patients and their families, providing clinical services independently and in collaboration with transdisciplinary teams.

About the Opportunity:

• Schedule: Monday to Friday
• Hours: 9am to 5pm
• Setting: Acute Care Medicine

Responsibilities:

• Conduct psychosocial assessments and develop individualized treatment plans
• Assist with hospital discharge planning and evaluate alternate care arrangements
• Provide counseling and concrete social services in both inpatient and NYC community settings
• Collaborate with medical teams to develop, implement, and evaluate treatment plans
• Make community referrals, assist patients in accessing services, and provide outreach
• Act as a mandated reporter and participate in court proceedings when necessary
• Document patient interactions, maintain accurate records, and prepare reports
• Participate in team meetings, committees, quality improvement activities, and training
• Ensure compliance with hospital policies, safety protocols, and regulatory standards
• Provide clinical services under supervision when required

Qualifications:

• Active New York State dedicated Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
• Prior experience in an Acute Care and/or Hospital setting
• Strong background in Discharge Planning and Patient Care Coordination
• Proficiency in Microsoft Word and Excel
• Excellent organizational, communication, and writing skills
• Ability to thrive in a fast-paced, high-volume healthcare environment
