About Job

Roles and Responsibilities

1. Provides comprehensive assessment and linkage to any and all social, supportive, housing, medical, recreational, spiritual and community-based resources that are available to the individual.
2. Provides case management services to consumers of Mamie Nichols apartments, including acting as liaison between housing organization and consumer and providing assistance with yearly or interim recertification as needed as well as maintaining occupancy.
3. Provides comprehensive assessment and referral to appropriate Behavioral Health and Drug and Alcohol services.
4. Provides assistance in all eligible entitlement programs.
5. Attends all veteran community provider meetings and collaborations to maintain partnerships with other veteran and community providers
6. Conducts outreach, develops and maintains relationships with community partners to obtain and maintain on-going referrals
7. Provides advocacy on behalf of the individual with regards to specific veteran’s service providers.
8. Provides assessment and identification of training and educational needs, along with the procurement of services that would meet those needs.
9. Provides assistance with enrollment in Vocational Training programs and educational training.
10. Provides assessment and development of a specific step by step plan to employment.
11. Provides crisis intervention services as needed.
12. Acts as liaison between various components of Veterans support and service delivery system, the Department of Behavioral Health and Intellectual Disabilities and CATCH Clinical and Residential Services.
13. Make home, hospital and community-based visits a minimum of every 14 days (or more often as determined by the consumer’s level of care).
14. Responsible for compliance with all Agency and Program record keeping and documentation as deemed appropriate by Agency and Program Policies and procedures.
Responsible for all other duties as assigned by Director.
Qualifications and Education Requirements
Master’s degree in social work with one to three years direct service experience. Excellent skills in assessment, documentation and communication. Knowledge of veteran’s needs, population and resources; veteran preferred.

WORK AUTHORIZATION/SECURITY CLEARANCE (IF APPLICABLE)
Must have an acceptable PA criminal history, FBI clearance and Child Abuse clearance done prior to hire.

WORK ENVIRONMENT
This job operates in a professional office environment. This role routinely uses standard office equipment such as laptop computers and smartphones.

POSITION TYPE/EXPECTED HOURS OF WORK
This is a full-time position. Days and hours of work are Monday through Friday, 8:30 a.m. to 5 p.m. This position may require long hours at certain times.

Must have physical ability to: (A) Climb stairs; (B) Sit for prolonged periods of time; (C) Carry heavy documents; (D) Bend and kneel.
