Job Summary
We are seeking a compassionate and dedicated Direct Support Counselor to provide essential support and assistance to individuals with disabilities or special needs in a home or community setting through Coordinated Family Services. The ideal candidate will play a crucial role in enhancing the quality of life for our clients by promoting independence, dignity, and respect. This position requires a strong commitment to patient care and adherence to HIPAA regulations. This role ensures that individuals with intellectual, developmental, and/or behavioral health needs are supported in achieving their personal goals while also receiving consistent case oversight, advocacy, and coordination of services. This position blends hands-on support with administrative and coordination duties.
Responsibilities
- Direct Support Counselor :
- Deliver person-centered, hands-on care and support based on each individual’s service or treatment plan.
- Assist with activities of daily living (ADLs), including hygiene, dressing, meal preparation, and medication assistance.
- Facilitate community integration, social skill development, and recreational outings.
- Monitor and report changes in individual behavior, health, or emotional well-being.
- Use trained de-escalation and behavior support strategies to manage challenging situations.
Case Management & Coordination:
- Serve as the primary point of contact for assigned individuals and their families.
- Coordinate and implement Individual Support Plans (ISPs) in collaboration with the interdisciplinary team.
- Maintain regular communication with individuals, families, and service providers.
- Facilitate service referrals, coordinate appointments, and ensure linkage to supports.
- Conduct home visits and progress check-ins as required.
- Complete all required documentation, including progress notes, incident reports, and service summaries.
- Track and manage authorizations and compliance with DDS regulations.
- Serve as an advocate for individual rights and service access.
Experience
- High school diploma or GED required.
- Minimum of two (2) years of experience providing direct services or case management to individuals with developmental disabilities or related populations.
- Must be at least 18 years of age.
- Valid driver’s license with clean driving record and reliable transportation.
- Ability to pass criminal background check, CORI, fingerprinting, and drug screening.
Preferred Qualifications:
- Associate’s or Bachelor’s degree in Human Services, Social Work, Psychology, or related field.
- Experience working within DDS programs or familiarity with DDS regulations and service standards.
- CPR/First Aid certification and Crisis Prevention/Intervention training.
- Experience with electronic health records (EHR) and case documentation.
