Salary Range: $93,190.50 - $100,493.14 per year (based on licensure and experience)

Position Overview

Sun River Health is seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our team at the Inwood clinic in the Bronx, NY. This full-time position offers an exciting opportunity to work within an interdisciplinary team, providing integrated care to patients facing various health and social challenges, including chronic illnesses, HIV, aging, and domestic violence. The LMSW will conduct biopsychosocial assessments, provide case management, deliver counseling services, and participate in crisis intervention—all while adhering to a patient-centered approach that aligns with Sun River Health’s values.

Key Responsibilities

Comprehensive Assessments & Care:

  • Conduct biopsychosocial assessments for new patients referred by primary care providers or other service providers, addressing both medical and psychosocial concerns.
  • Perform mental status exams and collaborate with the healthcare team, including psychiatrists and primary care providers, to create treatment plans and ensure comprehensive care.

Direct Counseling & Support

  • Provide individual, family, and group counseling services to patients and their families, particularly those impacted by chronic illness, HIV, domestic violence, and other psychosocial challenges.
  • Offer crisis intervention and conduct emergency evaluations for patients in immediate need.

Case Management

  • Coordinate case management services to ensure that patients receive the appropriate care, support, and referrals to additional specialists or community resources as needed.
  • Collaborate with external providers for psychiatric evaluations or other intensive services as required.

Collaboration & Teamwork

  • Work closely with the medical, behavioral health, and HIV care teams to develop and implement holistic treatment plans for patients.
  • Participate in interdisciplinary case conferences and contribute to integrated care strategies, including group therapy sessions.

Documentation & Compliance

  • Ensure all clinical documentation is completed accurately, on time, and in compliance with Sun River Health’s standards and regulatory requirements.
  • Maintain up-to-date case records and comply with all applicable grant and funding requirements.

Prevention & Education

  • Develop and implement prevention strategies for individuals at risk for HIV, chronic conditions, and other adverse health/social outcomes.
  • Support the community by facilitating access to resources and educating patients on self-management and health maintenance.

Qualifications

  • Education: Master’s degree in social work (MSW) from an accredited program.
  • Licensure: New York State Licensed Clinical Social Worker (LCSW) required.
  • Certifications: CPR/BLS certification required.

Experience

  • A minimum of 1-2 years of experience in social work, particularly in a healthcare setting, is preferred.
  • Experience working with underserved populations, individuals living with HIV, or those experiencing chronic illness, domestic violence, and aging issues is a plus.

Skills

  • Strong communication, interpersonal, and organizational skills.
  • Ability to work collaboratively within an interdisciplinary care team.
  • Knowledge of case management practices, crisis intervention, and mental health care.
