PURPOSE OF POSITION: Under the supervision of the Director of Social Work, the Psychiatric Social Worker performs psychosocial assessments, treatment plans, and discharge planning functions for all assigned patients. The Psychiatric Social Worker acts as a member of an interdisciplinary team, providing quality inpatient care and ensuring facilitation of appropriate aftercare.

Responsibilities:
- Review medical record, interview assigned patients, and perform psychosocial assessments and treatment plans for all new admissions to determine patient health care needs.
- Provide culturally competent direct service to patients and their families (if applicable) using a variety of social work modalities in order to help individuals maximize their functioning.
- Advocate on behalf of the individual to ensure maximum access to health care; assess barriers; and offer solutions.
- Provide interdisciplinary team with assessment findings and suggest potential impact on overall care of patient.
- Provide psychoeducation, counseling, and crisis intervention to patients and their families (if applicable).
- Formulate and implement treatment and discharge plans (while promoting patient choice) that are safe, timely, and reflective of Brunswick Hospital Center’s expectations of the site. Initiate communication with post-acute providers and payers as needed.
- Incorporate knowledge of community resources and entitlement programs in all planning.
- Maintain appropriate, professional patient care documentation as defined by Brunswick Hospital Center’s policies and procedures.
- Attend departmental meetings and represent department and profession in various hospital/site based committees and work groups as needed.
- May assign and monitor undergraduate and graduate level Social Work students as a Task Supervisor; assign cases to students and monitor work through regular supervisory meetings.
- Lead or co-lead psychotherapeutic groups when indicated.
- Collect data for the formulation of departmental reports as required.
- Other related tasks as assigned.

Required Qualifications:
- Speak, read and write English
- Master’s Degree in Social Work from an accredited institution
- Current, unrestricted license/certification from New York State (LMSW) required
Preferred Qualifications:
- 1-3 years previous experience in health care setting, preferably a hospital
- New York State LCSW
- SIFI certification a plus
