Overview
We are seeking a dedicated and compassionate Care Managers to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care, ensuring that individuals receive the highest quality of medical services tailored to their unique needs. This position requires a strong background inthe care of elderly or disabled persons.

Responsibilities

• Coordinate and manage patient care plans in collaboration with healthcare providers, patients, and families.
• Conduct comprehensive assessments of patients' medical histories and current health status.
• Conducting on-site visits with each member at the qualified setting bimonthly (alternating with the bimonthly visit by the registered nurse/csw per policy) for Level I, and monthly for Level II
• Monitor patient progress and adjust care plans as necessary based on evolving health conditions.
• Educate patients and families about treatment options, medical terminology, and care processes.
• Advocate for patients’ needs within the healthcare system to enhance their experience and outcomes.
• Stay informed about best practices in infectious disease care and emergency medicine to provide up-to-date guidance.
• Document all interactions and updates in compliance with healthcare regulations.
• Travel to member residences/meetings; will require transportation to get to various work sites

Experience

• Clinical experience in the care of elderly or disabled persons
• Familiarity with managed care systems and protocols is beneficial.
• Proficiency in medical terminology to effectively communicate with healthcare professionals and patients alike.
• Ability to work collaboratively within multidisciplinary teams while demonstrating strong interpersonal skills.
• Microsoft Office

Qualifications

• Bachelor's Degree (Mandatory)
• Social Worker License (Preferred)
• (2) years experience clinical/working experience in the care of elderly or disabled persons
• Current CPR
• Must be able to pass a background check
• Must have a Valid Drivers license and Transportation

Pay: $56,771.00 - $59,912.00 per year
