Day to *****
Scheduling and conducting face to face and/or telephonic member assessments
Coordinating member’s care (needs identified during assessments)
Completing all required paperwork within state specific turnaround times
Collaborating with member and their LAR to develop an individualized plan of care
Completing comprehensive daily/weekly status and handoff reports
Requirements
Minimum of LVN License or bachelors degree in Social Work or masters degree in Social Work
2+ Years of Experience in a Community Health setting
Ability to travel in El Paso, TX area to visit members in their homes or other settings such as community centers and hospitals
Reliable transportation with a valid driver's license & good driving record
Bilingual in English and Spanish
