Our client, a reputable organization that drives solution to transform the lives of people with disabilities, people in poverty and people suffering from substance abuse/addition, has an immediate need for an LPN Behavioral Health

Responsibilities:
Take vital signs, such as blood pressure, temperature and weight
Assist with diabetes medication and treatment
Take medical histories and enter information in computer systems
Coordinate with Case Managers, Senior Case Managers and Director of Social Services to ensure proper care, nutrition and treatment plans are followed
Follow up with community health care providers to ensure medical treatment plans are documented, monitored and followed
Have flexibility to work a schedule that includes evenings and weekends

Qualifications:
New York State Licensed Practical Nurse.
Basic knowledge of human services
Bilingual English/Spanish preferred but not required.
Knowledge of chemical dependency as a primary diagnosis.
Excellent written and verbal communication
Assessment and case management
Knowledge of Microsoft Office tools

Ability to provide crisis intervention utilizing good interpersonal skills with a high level of maturity and responsibility

Job Type: Contract

Pay: $38.00 - $40.00 per hour
