About Job

Active Home Health and Hospice is looking for a Full-Time Assistant Care Coordinator (CNA) who is professional and self-motivated to join our team. Pay is based upon experience and training.


Responsible to the Director of Clinical Services for the implementation, delivery, coordination, and supervision of Home Health C.N.A & Homemaker staff and services, consistent with company policies and procedures.


Qualifications:

High school graduate or equivalent (GED)

Current C.N.A License

Ability to work independently, make accurate, and at times quick judgments. Ability to supervise others appropriately. Ability to respond appropriately to crisis setting. Acceptance of and adaptability to different social, racial, cultural, and religious modes.

Minimum 2 years of experience as a C.N.A., preferred. Active patient contact within past three years preferred.


Responsibilities:

Supervise the planning, coordination & delivery of C.N.A and Homemaker services.

Assign patient/family to appropriate C.N.A and Homemaker staff memeber(s)

Participate and provide leadership in all IDT meetings, facilitating problem solving and delivery of team support.

Work with the Human Resources Manager and the Director of Clinical Services to hire all C.N.A's and Homemakers.

Participate in orientation and training of new C.N.A and Homemaker personnel.
