Job Summary:
Hospice agency is seeking a compassionate and experienced Medical Social Worker (MSW) to join our interdisciplinary hospice team. The MSW provides emotional support, psychosocial assessments, and community resource coordination for patients and families facing end-of-life care. This per-visit position is ideal for someone seeking flexibility and meaningful work in a supportive, patient-centered environment.
Responsibilities:
- Conduct psychosocial assessments for new hospice admissions.
- Provide counseling and emotional support to patients and families regarding end-of-life issues, grief, and loss.
- Develop individualized care plans in collaboration with the hospice interdisciplinary team.
- Assist families with advance directives, funeral planning, and accessing community resources and financial assistance programs.
- Maintain accurate and timely documentation in accordance with hospice policies and regulatory requirements.
- Participate in interdisciplinary team meetings and contribute to the overall plan of care.
- Advocate for patient rights and support ethical decision-making.
Qualifications:
- Master’s degree in Social Work (MSW) from an accredited school of social work.
- Minimum of one (1) year of experience in a healthcare or hospice setting preferred.
- Current state licensure as a Licensed Clinical Social Worker (LCSW) or Associate Clinical Social Worker (ASW), if applicable by state law.
- Valid driver’s license, auto insurance, and reliable transportation.
- Excellent communication, interpersonal, and organizational skills.
- Proficiency with electronic medical record (EMR) systems preferred.
Compensation:
- Pay Rate: $135 per completed, documented visit.
Pay: $135.00 per hour
Expected hours: 135 per week
Work Location: In person
