It is the mission of Columba Services to provide respectful and effective services to the individuals residing in the three Single Room Occupancy residences sponsored by Columba Kavanagh House. It is understood that the persons residing in these facilities have suffered extraordinary hardship and have significant challenges to living empowered healthy and productive lives. Therefore, beyond the normal concern for an individual's rights and freedoms, the responsibilities described herein shall be performed with the upmost diligence and care towards the goal of facilitating their further growth and independence.
The Case Manager/ Supervisor shall have a Master's degree in a social services-related field and two years of experience. Some of the experience must have been in a Supervisory and administrative capacity with homeless, homeless mentally ill, or mentally ill adults. Qualified individuals must demonstrate the capacity for leadership and the ability to supervise others. The Supervisor reports to the Director of Residential Services.
The Supervisor shall be responsible for the following tasks. Direct supervision of services to tenants. This responsibility shall include and not be limited to the development and implementation of service plans, maintenance and acceptable health and safety standards in the facility, the behavior and performance of staff in carrying out tasks and services, and facilitation of program improvements wherever possible. Services shall include these delivered those delivered directly and those available by community providers.
Leading the weekly program meetings (staff and rounds). These shall include collaborative conferences with other providers, service plan reviews, and any other such meeting where the services to Columba Kavanagh tenants are discussed. The Supervisor shall have the responsibility to be certain that all meetings are recorded in the proper manner and those recordings distributed as appropriate.
The management of referrals for services to Columba Services. This shall include scheduling intake meetings, consulting with the Director of Residential Services, Intake Coordinator, and others regarding decisions on referral materials, and facilitating intake decisions as appropriate.
