Job Description
Social Worker with MSW designation. Please only apply if you have an MSW and Hospice experience. The Social Worker will be responsible for the implementation of standards of care and assessments of psychosocial status of patients and families/caregivers to define goals and interventions to provide excellent end of life care. A Master's degree in Social Work is required.
Responsibilities and duties:
- Carries out social evaluations and plans interventions based on evaluation findings
- Counsels patients and family/caregivers as needed in relationship to to stress and other identified coping difficulties.
- Supports patients with their medical condition and life-limiting experience as it pertains to their existential, cultural/familial and spiritual experience
- Maintains client records on all patients
- Provides information and referral services
- Participates in the development of the individualized plan of care and attends regularly scheduled interdisciplinary group meetings
- Maintains collaborative relationships with IDG to support patient care
- Serves as liaison between patients and families/caregivers and IDG
- Adheres to professional and ethical practices
Qualifications and skills:
- Master's Degree in Social Work required
- Minimum one year experience in hospice or health care.
- Experience in hospice care preferred, health care considered
- Knowledge of Hospice and Palliative Care preferred
- Ability to distinguish and respond appropriately to the grief process
- Posses and maintains current CPR certification
- Must attend weekly IDG meetings on Wednesdays
- Experience with EMRs
- Bilingual skills are helpful
