We take a team approach to care that includes physicians, nurses, social workers, chaplains, hospice aides, volunteers, and bereavement counselors. We provide hospice service wherever patients and families need us: in their home, in a long-term care facility, or in our inpatient care unit that is based in the Texas Medical Center. Our team of doctors and nurses specialize in managing pain and symptoms that are unique to life-limiting illnesses.Summary: Candidate would be assigned to meet and exceed the census goals by proactively interacting with the Care Access Advocates and the professional community to attain referrals and admissions. Responsibilities to include:
  • Developing organization-wide care access strategies
  • Manage and motivate Care Access Advocates
  • Identify and develop community connections with the goal of expanding the reach of services provided by Houston Hospice
Qualifications:
  • Bachelor’s degree (BA/BS) in a healthcare-related field, marketing, or applicable field experience.
  • Two years experience in healthcare sales leadership.
  • Hospice experience required.
  • Experience with Managed Care, Medicare/Medicaid Advantage plans preferred.
  • Ability to handle detailed and confidential information.
  • PC experience and Microsoft Office software experience preferred.
  • Ability to work a flexible schedule. (infrequent evenings and weekends)
  • Effective interpersonal skills.
  • Excellent communication skills, both oral and written as well as superior presentation skills.
  • Excellent organizational skills to handle a variety of tasks simultaneously.
  • Ability to travel to multi-work locations during the workday and evening as needed.
  • Must have a reliable, insured vehicle and valid Texas Driver’s license.
  • Knowledge of applicable Federal/State laws governing and hospice licensure.
Duties/Responsibilities:
  • Analyze admissions to identify key physicians, hospitals, nursing facilities and other referral sources.
  • Work closely with Care Access Advocates in the field
  • Help Care Access Advocates develop contacts on a regular basis to maintain positive relationships with referral sources.
  • Help Care Access Advocates develop new referrals from groups such as physicians’ offices, healthcare professionals, discharge planners, nursing home professionals and Hospitals.
  • Monitor and report on marketing and admissions activities through regular use of Customer Relationship Management (CRM) tool.
  • Staffing and supervision of Care Access Advocates, including training, establishing goals and performing evaluations.
  • Help Identify and eliminate barriers to referrals and admissions.
  • Host meetings in hospice facilities and other venues for referral sources and potential referral sources.
  • Active collaboration with Care Access Manager.
  • Run weekly sales meeting.
  • Interact with internal colleagues to keep them informed of marketing activities and reports from referral sources.
  • Understands and adheres to all organizational Strategic and Operational Goals and Initiatives, as well as organizational privacy practices and compliance. programs.
  • Perform other duties as assigned.
