Job Overview
We are a home health agency seeking a dedicated and detail-oriented LPN Intake Coordinator to join our team. The ideal candidate will play a crucial role in managing patient intake processes, ensuring compliance with HIPAA regulations, and providing exceptional customer service in a medical office setting. This position requires strong organizational skills, proficiency in medical terminology, and the ability to effectively communicate with patients and healthcare professionals.
Responsibilities
- Receives and coordinates all referrals, which may include notification of insurance providers for prior authorization and services covered.
- Takes physician orders and documents information correctly.
- Assists prospective clients and referral sources in identifying appropriate services to meet the client’s home care needs.
- Provides a referral source with the company's scope of service, coverage area, etc.
- Ensures quality relations and maintains a professional, high-quality image with referral sources.
- Evaluates referral for appropriateness of requested services.
- Gathers client clinical and demographic information at the time of referral and inquiry.
- Enters referral information into the EMR software accurately.
- Communicates referral information to clinical manager or designee, clinicians, providers, and patient/caregiver.
- Assists with assigning a referral to the clinician for assessment and start of care.
- Maintains knowledge and education to remain current, efficient, and productive as a home health resource to referral sources.
- Maintains confidentiality of all information about clients, families, and employees.
- Maintains positive and effective communication with clients, referral sources, and office staff.
- Demonstrates self-direction to prioritize and accomplish job responsibilities.
- Using discretion and independent judgement in handling patient or physician complaints received, documents and forwards them to the appropriate administrative employees.
- Provide direct patient care as needed, as assigned by the Clinical Manager.
- Performs other related duties and responsibilities as assigned by the Clinical Manager or designee.
- Full-time office hours. No remote positions available.
- ICD-10 coding experience preferred.
Qualifications:
- Currently licensed as an LPN in the state of Ohio. The license must not have been revoked, suspended, or have limitations or restrictions.
- At least three years of health care experience, including intake and home health experience, are preferred. Knowledge of Medicare, private insurance, and Medicare Advantage plans is also required.
- Ability to promote and maintain a positive attitude and encourage others to do the same.
- Able to handle stressful people and sensitive situations.
- Excellent written and oral communication skills.
- Strong organizational skills and the ability to work independently with minimal supervision.
- Demonstrates ability to make appropriate clinical judgements regarding the referral process.
- Proficient with computers, portals, EMR, and able to learn new software.
- Knowledge of Word and Excel is a plus.
- Pass initial and ongoing background studies and screenings, including but not limited to Ohio's Department of Health and the Federal Office of the Inspector General’s List of Excluded Individuals and Entities.
- Must be a team player willing to go the extra mile for staff and patients.
