Your Role:

• Monitor patient progress and intervene when needed to support their health and well-being.
• Provide exceptional customer service over the phone, assisting patients with scheduling, inquiries, and follow-ups.
• Conduct patient assessments, educate them on self-care, and facilitate communication with care teams.
• Efficiently schedule appointments and optimize patient experience.
• Accurately document all patient interactions in a timely manner.

Job Requirements

MUST HAVES

• Strong customer service skills.
• 80% phone communication with patients.
• High School Diploma or equivalent.
• Own transportation and ability to travel to nearby PCP For Life locations.
• Availability to work at least one weekend day.

PREFERRED:

• Bilingual in English and Spanish.
• Certified Medical Assistant.
• Healthcare experience (preferred but not mandatory).
• Knowledge of eClinicalworks EMR (a plus).
• Effective verbal and written communication, customer relations, multi-tasking and organizational skills along with working with diverse populations, often in stressful fast-paced environments.

Salary: $14.00 - $17.00 per hour

Location: We’re hiring for multiple clinic locations in the Greater Houston area (Hybrid work environment). Exact placement will depend on clinic needs and your availability.
