Master's Degree Preferred in either Social Work or Counseling.
BA/BS degree in social services with 3+ years of documented experience working in clinical settings required.
Description:
The Utilization and Discharge Coordinator is responsible for providing care and treatment as outlined in the client’s treatment plan and functions as a member of the client’s treatment team as outlined in the policy and procedures manual.
- Actively participates in developing, implementing and coordinating the discharge goals on the treatment plan.
- Documents all therapeutic interactions and discharge planning activities in the client’s chart.
- Recognizes and reports signs and symptoms of abuse and neglect.
- Demonstrates knowledge and understanding of client needs based on the age and functioning level of the client.
- Consistently includes the parent/guardian in client care.
- Establishes and maintains contact with relevant community agencies and school personnel to provide appropriate linkage.
- Provides education to client and parent/guardian regarding special needs of the client according to the client’s abilities.
- Adjusts communication and aftercare plans to accommodate age and intellectual ability of the client and family.
- Involves client in decision making in regards to discharge planning.
- Actively participates in process improvement through data collection, evaluation of findings and improving the process.
- Client and parent/guardian are educated on community agencies and discharge options.
- Aftercare plans are individualized to each individual client’s needs.
- Provides regular consultation to treatment team regarding discharge planning.
- Coordinates with targeted case managers and SAMH the last 30 days of client’s treatment.
- Serve as liaison with aftercare school placement.
- Obtains signatures of all aftercare providers.
- Effective communication with therapists and nursing to provide accurate information to insurance providers to ensure continues authorization of funding
- Participates in TRT, FSBT and MDT meetings on behalf of client to coordinate aftercare services.
- Communicates with insurance providers as needed outside of reviews to ensure continued authorization.
- Models appropriate and professional behavior, i.e., dress, habits, personality, tact, etc.
- Provides instruction to the client regarding their length of stay at daniel.
- Disseminates information at departmental staff meetings.
- Consistently follows agency policies for information management and client records.
- Reports and completes incident reports in the time prescribed as per agency policy.
- Maintains accurate and organized filing of all documentation and information.
- Relates with clients, parents/guardians, peers and the general public in a respectful and professional manner.
- Actively supports agency philosophy and goals.

- Demonstrates ability to function independently to include follow through with assigned tasks.
