Job Overview
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will possess a strong background in providing support and assistance to individuals facing various challenges, including mental health issues, addiction, and senior care. This role requires a commitment to enhancing the well-being of clients through effective intervention strategies and comprehensive care plans.
Duties
- Conduct assessments to determine clients' needs and develop personalized treatment plans.
- Provide crisis intervention services to individuals experiencing acute distress.
- Offer psychotherapy and counseling services, utilizing techniques such as Cognitive Behavioral Therapy (CBT) to promote mental health.
- Document medical histories and maintain accurate medical documentation in compliance with ICD-9 standards.
- Collaborate with healthcare professionals in hospital settings to ensure comprehensive patient care.
- Facilitate addiction counseling sessions for individuals struggling with substance use disorders.
- Support seniors in community care settings, addressing their unique physical and emotional needs.
- Advocate for clients’ rights and connect them with necessary resources and support services.
- Maintain confidentiality and adhere to ethical standards in all interactions.
Requirements
- A degree in Social Work or a related field is required; licensure is preferred.
- Experience in patient care, crisis intervention, or addiction counseling is highly desirable.
- Familiarity with medical documentation practices, including ICD-9 coding, is advantageous.
- Strong interpersonal skills with the ability to build rapport with diverse populations.
- Excellent communication skills, both verbal and written.
- Ability to work collaboratively within a multidisciplinary team environment.

Pay: $96,000.00 - $136,000.00 per year
