- Patient Care Coordination: Develop, implement, and manage individualized care plans for patients with complex health needs. Collaborate with multidisciplinary teams to ensure comprehensive and effective care. Completes management functions including patient/family education. Refers reviewed cases to a Physician Advisor as needed.
- Assessment and Planning: Conduct thorough assessments of patients' medical, psychological, and social needs. Create and adjust care plans based on ongoing evaluations and patient progress.
- Patient Advocacy: Serve as a primary contact for patients and their families. Provide education on health management and resources, ensuring patients understand their care plans and treatment options.
- Case Management: Monitor and evaluate patient care and outcomes, coordinating with healthcare providers to address any issues or changes in patient condition. Ensure timely follow-up and continuity of care.
- Documentation: Maintain accurate and up-to-date records of patient interactions, care plans, and outcomes. Ensure compliance with regulatory requirements and organizational policies.
- Resource Management: Identify and facilitate access to community resources and support services for patients. Assist in managing care costs and optimizing the use of healthcare resources.
- Quality Improvement: Participate in quality improvement initiatives to enhance patient care and optimize processes within the care management program.

- Administrative Support: Assist with administrative duties related to care management, such as preparing reports and attending meetings.
- Training and Development: Contribute to the training and orientation of new staff members or interns as needed.
- Minimum of 3-5 years of experience in a clinical setting.
- Experience in managing complex cases and working with diverse patient populations is highly desirable.
- Preferred previous experience with case management and utilization management.
- Strong clinical assessment and critical thinking skills. Excellent communication, organizational, and interpersonal skills. Proficiency with electronic medical records (EMR) and care management software.
- Ability to work independently and as part of a team. Strong commitment to patient-centered care and continuous professional development.
- Current Registered Nurse (RN) license in the state of Illinois.
- Bachelor's Degree in Nursing (BSN) or higher from an accredited institution.
- Certification in Care Management (CCM) or similar certification is preferred.
