Registered Nurse - Home Health Care

Job Overview:
Axis Healthcare is seeking a compassionate and skilled Registered Nurse (RN) to join our team, providing direct care to patients in a home setting. As an RN specializing in home health, you will be responsible for evaluating and monitoring the recovery of elderly patients, ensuring effective treatment and rehabilitation. Your role will involve collaboration with our healthcare staff to ensure the best outcomes for our patients.

Role/Case-Specific Care:

• Wound care (LLE)
• Tube feeding: Continuous enteral feeding/medication administration
• Patient education
• Vital Sign Management

Key Responsibilities:

• Assess and treat patients with wounds or illnesses.
• Administer prescribed medications and manage the patient's overall care plan.
• Maintain accurate patient records and prepare necessary documentation for insurance claims.
• Collaborate with office staff to monitor patient recovery and outcomes.
• Educate patients on injury management, rehabilitation, and safety measures to prevent further injuries.
• Act as a liaison between patients, employers, and insurance companies to facilitate the workers' compensation process.

Qualifications:

• Active RN license in [TX].
• Previous experience in a home health setting.
• Strong communication and interpersonal skills.
• Ability to manage multiple tasks and work independently.
• Familiarity with workers' compensation regulations and case management practices is a plus.
• RN documentation on paper and electronic software.
