Clinical Social Worker (CSW) Job Duties and Responsibilities:

• Evaluating clients to determine their needs and the support options that can help them

• Creating individualized care plans for each client

• Referring clients to community resources to help their specific situations

• Collaborating with other care providers to coordinate services

• Monitoring clients

• Maintaining records of treatments and services

• Providing talk therapy

• Responding to crisis situations

• Additional job duties and responsibilities as assigned by job description, administrator, Director of Nursing (DON), and Regional support team.

• Licensed Clinical Social Worker (LCSW) is preferred but not required.
