JOB SUMMARY

The primary focus of this position is to provide case management services to families and individuals who are homeless and enrolled in the Rapid Re-Housing Program and individuals at risk of homelessness in the Homeless Prevention Program.

GENERAL RESPONSIBILITIES

● Interview and evaluate clients and families to complete the intake process.

● Utilize advanced knowledge to

○ Screen and assess eligibility for requested services for homeless families.

○ Complete intense assessment of child and family

○ Develop and implement a Family Promise Orientation program for families.

○ Analyze participants’ situations, capabilities, and problems to determine services

required to meet the program needs of participants.

○ Formulate, implement, evaluate and revise goal-oriented and strength-based family

action plan.

○ Establish and implement family program expectations.

○ Provide timely crisis intervention to ensure family success.

○ Develop a crisis/safety exit plan for the family.

○ Exercise judgment to intervene as an advocate for clients in emergency and crisis

situations.

● Formulate daily contacts into the database and meet all deadlines of documentation including assessments, action plans, crisis plans, monthly reports, and evaluations.

● Effectively engage families to promote change to improve family functioning and overall home environment upon exit of the program.

● Provide information and referral services to help families access other community resources.

● Work in collaboration with other providers, school personnel, mental health professionals, and the Department of Human Services. Attend meetings as necessary.

● Complete proper documentation and file information as outlined by programmatic procedures, including all required reports for federal and local funding.

● Work in collaboration with landlords and property management companies who are housing program participants.

● Work on developing positive working relationships with landlords and property managers in the county.

● Coordinate with Shelter Case Manager to provide housing assistance to clients that are moving out of the Portlight Shelter.

● Participate in special events.

● Safely transport clients as needed.

● Other duties as assigned

QUALIFICATIONS AND EDUCATION REQUIREMENTS

● Bachelor's Degree in Social Work, Human Service, Psychology, Counseling, or another related field

● At a minimum, one year of experience working with disadvantaged families.

● Accurate and complete recording of data

● Excellent Computer skills: Internet, MS Word, MS Access, and MS Excel with experience in the Wisconsin Homeless Management Information System or similar intake program

● Experience working with the following issues: mental health, substance abuse, variety of behaviors, low income and/or homeless populations, and health care

KNOWLEDGE, SKILLS AND ABILITIES

● Ability to read, speak and write clearly in English.

● Ability to exercise good judgment and discretion in establishing a work schedule.

● Must work cooperatively and collaboratively with Workforce Development Center, OZ Human Services, and other community agencies.

● Must establish and maintain productive and professional relationships with guests, colleagues, and other community agencies.

● Must maintain client confidentiality.

● Ability to prioritize and make appropriate decisions.

● Ability to work independently and effectively manage time.

● Must possess strong oral and written communication skills.

● Ability to provide culturally, ethnically, and gender-sensitive services.

● Ability to follow directions and implement recommendations.

● Ability to identify problems and solutions.

● Ability to self-motivate and remain on task.

● Knowledge and compliance with policies and procedures as detailed in the personnel policy and procedure manual.

● Ability to operate standard office equipment including telephone, fax, copy machine, shredder, computer, and various software programs.

● Ability to promptly and adequately maintain client records in various electronic database systems.

● Ability to operate a motor vehicle safely and obey all traffic laws. Must have reliable transportation daily, a valid driver’s license, and required automobile insurance.

OTHER INFORMATION

● Occasional evening and weekend hours are required based on the program's needs.

● Maintain a caseload as assigned.

● Ability to sit, stand and drive throughout the day.

● Ability to tolerate environmental conditions such as air conditioning, dust, or odors.

● Ability to lift and move objects weighing up to 50 pounds.

● Must possess visual acuity.

Job Type: Full-time

Pay: $45,000.00 - $47,500.00 per year
