Position Overview:
The Housing Case Manager assists individuals with disabilities in locating, securing, and maintaining adequate and affordable housing. This role focuses on identifying housing barriers, developing individualized housing plans, and supporting clients throughout the transition to stable community-based living.
Key Responsibilities:
- Develop and implement individualized housing plans with clients.
- Assist clients in locating appropriate and affordable housing options.
- Identify barriers to housing and help clients navigate those challenges.
- Provide ongoing support after housing placement to promote housing stability.
- Conduct assessments, home visits, and regular check-ins.
- Maintain accurate case documentation and comply with program reporting requirements.
- Coordinate with landlords, service providers, and other agencies on behalf of clients.
Qualifications:
- Bachelor’s degree in Human Services, Social Work, or related field — or equivalent experience (Required)
- Experience in housing access, case management, or disability services (Preferred)
- Familiarity with housing programs, public benefits, and local resources in the Twin Cities (Required)
- Experience working with vulnerable adults (Required)
- Strong organizational and communication skills
- Proficient in using case management systems and software (Required)
- Valid driver’s license and ability to transport clients (Required)
Job Types: Full-time, Part-time
Pay: $25.49 - $27.68 per hour
Expected hours: 25 – 30 per week
