Medical Social Worker
Duties:
The Hemodialysis Medical Social Worker facilitates the transplant process and establishes relationships with procurement organizations to ensure patients accessibility and preparedness for transplantation; coordinates referrals and required testing and evaluations for patients in preparation for transplantation; facilitates transient and permanent transfers, performs mental health and psychosocial evaluations and support for all dialysis patients and donors; works collaboratively with the Unit’s interdisciplinary team. Performs other duties within scope of practice as deemed necessary in the provision of care. This person assumes responsibility and accountability for the duties of the position and is cognizantof the philosophy, standards, objectives, and policies of the Schneider Regional Medical Center and their respective Department Any combination of a Master’s degree in Social Work or related field from an accredited program along with a minimum of three years of social work experience; License in Clinical Social Work and current VI licensure required;a minimum of two years experience working with patients with renal failure preferred. .
- Document patient interactions and progress in medical records, ensuring accurate coding using ICD-10
- Strong communication and interpersonal skills to effectively engage with patients, families, and healthcare professionals
- Ability to work independently as well as part of a multidisciplinary team
- Compassion, empathy, and a commitment to providing high-quality care
