Home Health Social Worker Overview:

Your Role and Responsibilities as a Home Health Social Worker:
- Assess the psychosocial status; establishing, monitoring, and delivering care as it relates to meeting the psychosocial needs of the patient, as directed by the physician’s plan of care
- Work with a team of MDs, RNs, CNAs, PTs, OTs and SLPs to provide excellent patient care
- Participates in ongoing interdisciplinary assessment of the patient and develops the plan of care in consultation with team members
- Completes and submits timely documentation addressing the treatment and interventions provided, and the patients response to care provided, including progress towards goals in accordance with professional standards of practice
Qualifications for an Interim HealthCare® Speech Language Pathologist:
- Master’s or Doctoral degree in social work from a school accredited by the Council on Social Work Education
- Active Illinois Social Worker License to practice
- Minimum 1 year of social work experience, preferrable in a home healthcare setting
- Current CPR certification
