Care Coordinator – Home Care Agency

About the Role:

At Hummingbird Care Services, we’re redefining what high-quality non-medical home care looks like. As a Care Coordinator, you’ll be the vital link between our clients, their families, and our caregivers—ensuring that care is delivered with compassion, professionalism, and consistency.

This is not a clinical nursing role. It is a fast-paced, client-focused administrative position ideal for someone who thrives on relationship-building, problem-solving, and staying organized.

Key Responsibilities:

• Serve as the main point of contact for a caseload of clients and caregivers
• Coordinate caregiver schedules and ensure consistency of care
• Conduct in-home assessments and create/update care plans
• Monitor client satisfaction and follow up regularly on care quality
• Communicate with families regarding care updates or concerns
• Accurately document client and caregiver communications in EHR
• Support onboarding of new clients and orientation for assigned caregivers
• Participate in weekly team meetings and contribute to collaborative care planning
• Share rotating after-hours on-call responsibilities with the care team

What We’re Looking For:

• 2+ years of experience in home care, case management, or customer service
• Excellent communication and organizational skills
• Empathetic, reliable, and proactive problem-solver
• Comfortable working with technology (experience with AxisCare is a plus)
• Familiarity with non-medical senior care, dementia care, or aging-in-place models
• CNA, LPN, or RN credentials are helpful but not required

Pay: $18.00 - $20.00 per hour

Expected hours: 40 per week
