What You'll Be Doing

Wake County Health and Human Services' Senior and Adult Services Division provides home- and community-based services to seniors, adults with disabilities, and their families. The goal is to promote self-sufficiency, safety, health, and independence—allowing individuals to remain safely in their homes or community and improving quality of life for residents in adult and family care homes.

The Senior and Adult Services Social Worker delivers case management and support services to adults in the community, including eligibility evaluations, referrals, and ongoing assistance to meet essential needs.

Essential Duties And Responsibilities

• Assess eligibility for the Special Assistance In-Home (SA-IH) program, which supports individuals at risk of adult care home or group home placement
• Provide case management and coordinate resources to meet clients' unmet health and safety needs
• Offer short- or long-term support to individuals not eligible for Adult Protective Services but still requiring assistance
• Serve elderly and disabled adults with limited income or support systems who need help accessing services
• Document all case activities and decisions in accordance with program requirements
• Act as the Targeted Housing Program Referral Agency contact, assisting clients in securing access to safe, affordable housing

Required

The Basics (Required Education and Experience)

• Bachelor's degree in Social Work or related field
• One year of job-related experience
• Valid Driver's License and "safe" driving record
• Equivalent education and experience are accepted

Beyond the Basics (Preferred Education and Experience)

• Experience working with elderly, disabled and adults with mental illness, intellectual developmental disabilities and substance abuse

Emergency Service Worker Statement
