Job Category: Care ManagementJob Description:The Care Manager LMFT will function as a core member of a multidisciplinary Oncology care team through a structured Care Management LMFT program in which outreach, education, care coordination and periodic follow up are provided. Acts as an advocate and helps patient gain access to resources while confronting their personal issues related to cancer diagnoses and survivorship. The position takes responsibility for management of a therapeutic caseload (individual therapy to adults) providing timely charting and record keeping. Master's Degree Marriage and Family Therapy or Behavioral Science.
Bachelor's Degree.
Upon hire: California Clinical Social Work Associate License. Or
Upon hire: California Marriage and Family Therapist License. Or
Upon hire: California Clinical Social Worker License. Or
Upon hire: California Marriage and Family Therapist License. Or
Upon hire: California Marriage and Family Therapist Intern License. Or
Upon hire: California Psychologist License.
