Duties

• Conduct comprehensive patient assessments to identify needs and develop individualized care plans.
• Provide clinical counseling and behavioral therapy to clients facing mental health issues.
• Facilitate group therapy sessions to promote social interaction and support among clients.
• Offer guidance on aging issues and geriatrics, ensuring that elderly clients receive appropriate resources and care.
• Collaborate with healthcare professionals to coordinate home community care services for clients.
• Perform diagnostic evaluations to determine the appropriate interventions for clients.
• Maintain accurate records of client interactions, treatment plans, and progress notes in compliance with legal and ethical standards.
• Advocate for clients’ rights and access to necessary services within the community.

Required Qualifications:

Master's Degree Social Work from accredited university.

Coursework/Training Courses or continuing education in Evidence-Based Intervention.

1 year social work experience in a health care setting (outpatient/acute care or skilled nursing facility).

Preferred Qualifications:

upon hire: Clinical Social Worker License (LCSW).
