The Endocrinology Clinic provides a wide range of services for a endocrine needs including subspecialty programs for diabetes, thyroid disease, and reproductive health patients. Our dedicated team of providers, nurses, social workers, pharmacists, and other health care givers care for the whole patient and use resources from the community to assist with ensuring the patient has what they need to improve their health.

What You Will Do As a Care Coordinator

  • Manages referrals, hospital discharge coordination, working with providers to complete Care Plans as necessary, creating goals with patients and follow-up with the patients on goal setting, and other duties for care coordination
  • Identify and manage daily work pertaining to Insurance issues, prior authorization, form completion and adherence concerns.
  • Assists with coordination of transition of care from pediatrics to adult care
  • Assist manager with Quality Improvement and safety initiatives for diabetes patients
  • Review referrals to the Endocrinology specialty and determine if patient meets criteria for the disease program.
  • Hypothyroid and Diabetes Graduation coordination - assist with scheduling reminders, ensuring patients have a PCP and follow up appt post-graduation from Endocrine clinic.
  • Provide technical expertise with Diabetes technology and interfaces.
  • Managing Glooko list to promote financial accuracy.

What You Will Need

  • High School Diploma or equivalent
  • Basic health care insurance coverage knowledge
  • 2+ years experience in a healthcare setting
  • Some knowledge of EMR software

Hours: 1.0 FTE

Compensation

22.47 - 32.58 USD Hourly
