Position Summary:

We are seeking a compassionate and experienced Long Term Care Nurse Specialist to join our team. This is a fully field-based role responsible for visiting members in their homes across Brooklyn or Bronx to conduct comprehensive assessments, recommend care plans, and ensure appropriate service delivery. Ideal candidates will have experience with MLTC, MAP, or Maximus, along with strong critical thinking and problem-solving skills.

Key Responsibilities:

• Prepare for each member visit by reviewing the EMR and prior assessments in advance.
• Conduct in-home visits to complete thorough assessments using provided documentation templates.
• Accurately document findings in real-time and load all required supplemental tools and forms.
• Use Lenavi system to review, validate, and correct documentation inconsistencies.
• Manage 2–5 visits daily, depending on type and length of required assessments.
• Flexibly adapt to changing schedules, travel demands, weather conditions, and process updates.
• Remain respectful and non-judgmental in diverse member home environments, including those involving adverse conditions or events.
• Interface with prospective and current members, family members, facility staff, and internal care management teams.
• Provide additional telephonic support as needed and attend quarterly in-office meetings.

Performance Expectations:

• Complete all documentation with accuracy and in a timely manner.
• Maintain strong communication through Outlook and Microsoft Teams.
• Demonstrate flexibility, adaptability, and professionalism in the field.
• Complete assigned visits daily (2–5 based on complexity).
• Engage in continuous coordination with the Care Management team and related departments.

Preferred Background & Experience:

• Required:
• Current RN license – New York State
• NY State Driver’s License with reliable transportation
• Experience with MLTC, MAP, or Maximus
• At least 2 years of experience in Discharge planning, or Long-Term Care
• Familiarity with UAS assessment tools
• Strong technology proficiency: Outlook, Word, Excel, Adobe, EMS
• Preferred:
• BSN
• Organized and detail-oriented with excellent verbal/written communication skills
• High flexibility and ability to work independently in the field

Job Types: Full-time, Contract

Pay: $55.65 - $61.09 per hour

Expected hours: No less than 40 per week
