If Yes, Here’s How You Will Contribute to the Team:
As a Patient Care Coordinator, you will:
- Coordinate services for patients transitioning from hospital or related facilities to home health care.
- Support the development and implementation of patient care plans post-discharge, ensuring alignment with physician directives and agency requirements.
- Collaborate with hospital social services and discharge planning teams to facilitate home health services.

Ensure adherence to regulatory requirements related to admissions and referrals
Manage the admission process with referral sources and the sales team.
Obtain necessary authorizations for home health services from insurance provide
- Assign caregivers to patients based on care plans and service requests.
- Provide caregivers with accurate instructions and information regarding patient needs.
- Report scheduling changes to the Director of Intake & Staffing and notify patients of any modifications to their care schedule.
- Assist in monitoring staff performance, including dependability and responsiveness
- Maintain accurate and up-to-date records of patient services, staffing, and scheduling.
- Complete and manage documentation, using both paper and electronic systems.
- Monitor overtime and mileage records, and address emergency scheduling needs as required.
- Provide timely notifications of schedules and changes to employees, patients, contract agencies, and supervisors.
- Address and resolve issues related to patient care and staff scheduling.
- Adhere to HIPAA regulations to ensure patient confidentiality and privacy
Must Have:
- High school diploma or equivalent
- 1 year of scheduling experience in home health, skilled nursing facilities (SNF), or a healthcare setting.
- Active Certified Nursing Assistant (CNA) certification in the state
- Willing to work beyond normal working hours, weekends, holidays, and/or on other shifts and in other positions when necessary
- Must be knowledgeable of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care
- Knowledge of Medicare rules and regulations, including JCAHO standards
Nice to Have:
Related experience in a long-term care, healthcare, or skilled nursing environment
